Army college drop formal statement?

i need an example of an already completed and accepted formal statement that someone has written. my PL keeps kicking my statement back even though ive had 4 different nco;s with college educations proof read and help me write it. i would love it if someone who has already gotten a drop could send me a copy so i have something to work off of. thanks
